(A) No amount of extensive discussion is going to clarify the customers' needs, since there are implied needs that cannot be expected to be stated by the customer. What should be required is creation of user acceptance tests (or customer acceptance tests).
(B) This is against the Agile Manifesto of having interactions over processes. A much better solution would be to create customer acceptance tests.
(C) Clearly, the only way developers can confirm their work is by running it through customer specified acceptance tests. This is the only acceptable way to confirm that the work done is as per the customers request.
(D) Even documenting extensively will not uncover all the needs the customers want, as much as working with a prototype. Therefore the developers focus should be to deliver something quickly that the customer can test out to see if their needs are met.
